This trial evaluates the Medtronic Intrepid Transcatheter Mitral Valve Replacement System TMVR for patients with moderate to severe or severe symptomatic mitral valve regurgitation who are not suitable for approved transcatheter repair or conventional mitral valve surgery. It is a multi-center, global, prospective, non-randomized interventional study where all participants receive the study device. The trial is sponsored by Medtronic Cardiovascular and focuses on treatment outcomes with this device. Participants receive treatment with the Medtronic Intrepid TMVR device designed to replace the mitral valve via a catheter procedure. There are two experimental groups a primary cohort and a cohort with mitral annular calcification, both receiving the TMVR device. This is a single-group study without randomization or blinding, and treatment is delivered through the transcatheter valve replacement system. During the study, participants will be monitored over time with assessments including mortality, stroke, kidney injury, wound infection, reoperation, and bleeding within 30 days or hospital discharge. Additional measures include the degree of mitral regurgitation at 6 months, quality of life improvements at 3 months, heart function status at 30 days, echocardiographic evaluations at 1 year, and cardiovascular hospitalizations over 1 year. The primary outcome is evaluated at 1 year, with follow-up continuing up to the studys completion in 2036.

This table compares participation in this trial with other options available for this condition. The information is drawn from published research and the trial’s study details and is provided for educational purposes only.
| Factors | Medtronic Intrepid Transcatheter Mitral Valve Replacement (TMVR) | Current Care: Surgical Mitral Valve Replacement (SMVR) or Standard Medical Therapy |
|---|---|---|
| What it is | A minimally invasive device procedure using the Medtronic Intrepid TMVR system to replace the mitral valve in patients with severe symptomatic mitral regurgitation who are unsuitable for surgery or transcatheter repair. | Guideline-supported management for severe mitral regurgitation: conventional surgical mitral valve replacement (SMVR) when feasible; otherwise, optimized medical therapy for those unsuitable for surgery. |
| Role in trial | Investigational replacement device evaluated for procedural success, safety, and clinical outcomes. | Not tested in this trial; represents established treatment options outside the study. |
| Primary use | Treatment of severe symptomatic mitral regurgitation in patients not suitable for surgery or transcatheter repair; aims to reduce symptoms and improve heart function. | SMVR is used to correct mitral regurgitation and improve symptoms and outcomes when patients are eligible; medical therapy is used to relieve symptoms in patients not suitable for surgery. |
| How it is given or performed | Device is implanted via a minimally invasive transcatheter approach (typically transapical or transseptal) under general anesthesia. | Surgical mitral valve replacement is performed by open-heart surgery under general anesthesia; medical therapy involves ongoing oral medications and monitoring. |
| Evidence status | Five-year data for earlier-generation Intrepid TMVR devices show feasibility and durable valve function, but limited safety and outcomes evidence for later-generation systems; ongoing trial is designed to address these gaps. | Surgical mitral valve replacement has robust evidence supporting symptom improvement and survival benefit; medical therapy less effective at correcting regurgitation but palliates symptoms. |
| Important limitations | Long-term safety and comparative benefit versus standard surgery are not established; device suitability may be limited by anatomy and procedural risk. | Surgical mitral valve replacement carries operative risk, may not be feasible for frail or anatomically complex patients; medical therapy does not correct the underlying valve malfunction. |
