What this Trial Is About
Researchers are evaluating factors that affect blood pressure and related cardiovascular health outcomes in adults with elevated blood pressure attending the Wayne Health Mobile Health Unit in Detroit and Cleveland. This observational study is part of the ACHIEVE GREATER Center, which aims to improve cardiometabolic health by addressing chronic conditions like hypertension, heart failure, and coronary heart disease. It focuses especially on understanding social determinants of health that impact blood pressure control and access to care in urban communities. The study includes two parts: a retrospective analysis of past Mobile Health Unit patients and a prospective observational cohort of patients consenting to ongoing follow-up. A subset of participants will be randomly assigned to receive text message reminders to take their blood pressure, while others will not. This nested randomized trial evaluates adherence to blood pressure monitoring protocols and subsequent health outcomes over 12 months. Participants will be followed through electronic health record review to track blood pressure levels, medication use, clinic visits, and cardiovascular events. The study team will assess how social and personal factors relate to blood pressure control and healthcare engagement. Outcomes include blood pressure measurement adherence, emergency department use, and achievement of blood pressure goals. The study will last at least 12 months, with findings aimed at improving mobile health programs and reducing health disparities.
