Eligibility Criteria

Eligible

You may qualify if you...

  • Aged 18 -70 years.
  • Diagnosis of hypertension with systolic blood pressure 6450 mmHg at the most recent clinic visit without diabetes.
  • Hypertension treatment naive or on monotherapy.
  • Receives primary care at one of the participating sites.
  • Willing and able to provide informed consent in English, Twi, Dagbani, Ewe, Hausa, or Yoruba.
  • Owns a smartphone (Android or iOS).
Not Eligible

You will not qualify if you...

  • Age under 18 years or over 70 years.
  • Severely elevated blood pressure (65180/110 mmHg).
  • Diagnosis of stroke, coronary artery disease, or kidney disease.
  • Diagnosis of end-stage renal disease (ESRD) treated with dialysis.
  • Serious medical condition limiting life expectancy or requiring active management (e.g., cancer).
  • Cognitive impairment or other conditions preventing study participation.
  • Pregnant or currently nursing a child.
  • Unwilling to provide informed consent.

Outpatient Treatment

Participants take part in the ADHINCRA Program, a nurse-led intervention that includes home blood pressure monitoring, use of a mobile health app, lifestyle counselling, and a simplified hypertension treatment protocol to manage blood pressure.

Visits occur depending on cohort start time with follow-up assessments at baseline, 12 months, and 24 months post-randomization
