What this Trial Is About
Researchers are evaluating the impact of adding an ultrasound-guided bilateral intermediate cervical plexus block to total intravenous anesthesia TIVA on the surgical stress response in patients undergoing total thyroidectomy. This study involves 48 adult patients classified as ASA I or II with euthyroid status, divided into two groups one receiving only TIVA and the other receiving TIVA combined with the cervical plexus block. The goal is to see if this combination reduces stress and inflammation during and after surgery, measured by biomarkers in saliva and blood. Participants will receive either standard TIVA or TIVA plus a bilateral intermediate cervical plexus block using 0.25% levobupivacaine. Before surgery, blood and saliva samples will be collected to establish baseline stress and inflammatory markers. During surgery, anesthesia depth and hemodynamic responses will be closely monitored, with pain management protocols standardized across groups. Postoperatively, various assessments including pain, sedation, and vital signs will be conducted, and additional saliva and blood samples will be taken to track biomarker changes over 48 hours. Throughout the study, participants will undergo blood and saliva sampling at multiple time points before and after surgery, with continuous monitoring of vital signs and pain levels. Researchers will record analgesic consumption and sedation scores, using tools like the Visual Analog Scale for pain and the Richmond Agitation-Sedation Scale. The study uses randomization and single blinding to compare outcomes between groups, aiming to provide evidence for improving anesthesia protocols that may reduce surgical stress and aid recovery. Total participation lasts through the immediate postoperative period with follow-up assessments up to 48 hours after surgery.
