Bed Side Assessment in Patients With Acute Respiratory Failure Under Invasive Mechanical Ventilation

This research aims to better understand respiratory mechanics in patients who require invasive mechanical ventilation due to acute respiratory failure. The study focuses on measuring lung instability, which refers to lung tissue that opens during inhalation and collapses during exhalation. This instability tends to increase with lung disease, such as viral pneumonia, and is assessed by analyzing ventilator data. The study evaluates lung instability by analyzing the low flow pressure-volume loop during ventilation, a quick and safe technique used in clinical practice. Researchers will measure lung hysteresis, a feature of this loop, to quantify instability. They will also record classic respiratory mechanics parameters like lung stiffness, gas exchange data including blood oxygen and carbon dioxide levels, biometric data such as height and weight, and imaging studies like CT scans, lung ultrasound, and electrical impedance tomography. Participants will undergo data collection related to respiratory mechanics and lung function over a period of up to 45 months. The primary outcome is the statistical prevalence of respiratory instability based on lung hysteresis at specific ventilator settings. Secondary outcomes include descriptions of classical respiratory mechanics associated with lung instability. This observational study involves monitoring participants without altering their treatment during the study period.
