Eligibility Criteria
You may qualify if you...
- At least 2 chronic conditions
- At least 1 functional limitation
- Intention to set at least 1 physical activity goal
- Live within 15 miles of University of Oklahoma Schusterman Center
You will not qualify if you...
- Life expectancy less than 6 months
- Progressive neurological condition
- In active treatment for cancer
- Not English speaking
- Cognitive impairment
- Current or history of severe mental illness
- In concurrent treatment with occupational and physical therapy

Duration - 10 weeks
Participants receive a 10-week behavioral activation and occupational therapy program, delivered in their homes by a trained occupational therapist. This includes setting and working on daily health self-management goals, with 1 session per week.
Weekly visits for up to 10 weeks (in-person sessions at home)
Duration - 12 weeks after treatment
Participants complete follow-up assessments at 10 weeks and 22 weeks after baseline to evaluate changes in health self-management and other outcomes.
