Eligibility Criteria

Eligible

You may qualify if you...

  • Adults aged 18 years or older
  • Have two or more chronic medical conditions from a defined list of 24 conditions
  • Have one or more gaps in evidence-based care, such as missed appointments or medication non-adherence
  • Have at least one self-reported social-related barrier to care, such as transportation, food, housing, utilities, or financial insecurity
Not Eligible

You will not qualify if you...

  • Major cognitive barriers such as dementia or psychosis
  • Multiple hospitalizations in the prior year (extremely high utilizers)

Your Study Journey

Screening

Duration - 2 to 4 weeks

Participants are screened for eligibility to participate in the trial.

1 visit (in-person or remote)

Outreach Intervention

Duration - Up to 3 months

Participants receive either higher intensity telephonic outreach from a health navigator with follow-up contacts for up to 3 months or lower intensity outreach via text, email, and/or letter with information about available benefits and services.

Multiple contacts depending on assigned outreach strategy

Follow-up Monitoring

Duration - Approximately 9 months after intervention

Participants are monitored for receipt of social services, reduction in social needs, and closure of clinical care gaps over time.

Assessments at approximately 6 and 12 months after enrollment
