Researchers will measure how well the system is established, its impact on individual and community health levels, and the feasibility of replicating it, including cost and qualitative assessments, over up to two years. Participants must live in the Jacksonville, Florida area and be willing to consent. The study includes adult Black individuals aged 21 to 80 years, with some groups specifically between 30 and 80 years old, and involves ongoing evaluation of prostate health equity outcomes.

Eligibility Criteria
You may qualify if you...
- Participants must be between 21 and 80 years old for AIMS 1 and 3, or between 30 and 80 years old for AIM 2
- Must be able to complete surveys in English
- Must live in Jacksonville, Florida or surrounding counties
- Must be willing and able to consent to the study
- For AIMS 1 and 3: Must be key personnel such as Community PI, CoLLab Program Manager, Site Managers, CAB members, or Community Health Workers
- For AIM 2: Must be a Black adult of any gender
You will not qualify if you...
- Younger than 21 or older than 80 years for AIMS 1 and 3
- Not a resident of Northeast Florida for AIMS 1 and 3
- Unwilling or unable to consent for AIMS 1 and 3
- Younger than 30 or older than 80 years for AIM 2
- Not a resident of Northeast Florida for AIM 2
- Unwilling or unable to consent for AIM 2

Participants access the CoLLab Learning Health System to use available services at least twice monthly while completing questionnaires to assess the impact and feasibility of the program.
At least 2 service visits per month plus periodic questionnaire completion

Ongoing evaluation of the feasibility of replicating the CoLLab Learning Health System in Black communities nationally, including cost analysis and qualitative assessments.
