Eligibility Criteria
You may qualify if you...
- Age 18 or older
- Eligible for Wisconsin Medicaid and planning to enroll before release
- History of substance use, such as considering alcohol or drug use a problem
- Living with or history of a chronic medical or psychiatric condition other than substance use disorder, including diagnosis or current prescription treatment
- Able to read, understand, and speak English

- Ineligible for Wisconsin Medicaid
- No history of substance use
- Not living with or no history of chronic medical or psychiatric condition
