You may qualify if you...

  • Age 18 or older
  • Eligible for Wisconsin Medicaid and planning to enroll before release
  • History of substance use, such as considering alcohol or drug use a problem
  • Living with or history of a chronic medical or psychiatric condition other than substance use disorder, including diagnosis or current prescription treatment
  • Able to read, understand, and speak English

  • Ineligible for Wisconsin Medicaid
  • No history of substance use
  • Not living with or no history of chronic medical or psychiatric condition
