Participants will receive weekly reminders and program materials, including worksheets and videos, in formats they prefer. During the study, participants will complete 8 short questionnaires about their mental health, coping skills, anxiety, resilience, sleep, pain, anger, and overall wellbeing before starting the program, at 6 weeks, and after the 12-week program ends. They will also complete a feedback questionnaire at the study's conclusion. The total time commitment is approximately 36 to 39 hours, including sessions, homework, and assessments. Researchers will monitor the feasibility of the DBT program and assess changes in mental health symptoms and participants' experiences.

Who Can Participate
Eligibility Criteria
You may qualify if you...
- Age over 18 years old
- Receiving care in the Manitoba Kidney Health Program at Seven Oaks General Hospital or Health Sciences Centre
- Either planning to start facility-based hemodialysis within 1 month or started within the last 90 days
- Able and willing to attend weekly DBT sessions in person or virtually
- Experiencing self-reported psychological distress related to advanced kidney disease or hemodialysis
- Deemed eligible by the study clinical psychologist
You will not qualify if you...
- Unable to complete the program due to planned relocation, transplant, or change in treatment
- Unable to communicate in English
- Unable to participate fully due to active psychiatric care with diagnosis
- Requiring psychiatric referral as assessed by care providers
- Experiencing current psychotic symptoms
- Cognitive impairment limiting group participation or homework completion
- Safety risk to staff such as physical aggression
- Diagnosed antisocial personality disorder or other factors limiting group participation
- Severe substance use disorder (referral offered if present)
