Participants will be tracked for key outcomes including mortality, hospital readmissions, major adverse cardiac events, and major adverse kidney events at 30 days and one year. Data collection includes reviewing patient records and outcomes to identify predictors of these events. The study aims to provide important information that can help improve care strategies for patients with combined heart and kidney disease over time.
