What this Trial Is About
Researchers are evaluating the use of enalapril compared to standard blood pressure medicines in postpartum women who had hypertensive disorders during pregnancy, such as preeclampsia or gestational hypertension. This study aims to learn if women are willing to be randomly assigned to these treatments after delivery and how these medications affect their blood pressure. The study addresses the important risk of chronic hypertension and cardiovascular disease in women after pregnancy-related high blood pressure conditions. Participants will be randomly assigned to receive either enalapril, an anti-hypertensive drug with potential heart-protective effects and a favorable safety profile during breastfeeding, or standard treatments like labetalol or nifedipine. These treatments are taken daily or twice daily, and the study will take place within a remote blood pressure management program. The study is designed as a phase 3 randomized controlled trial. During the study, participants blood pressure and medication use will be monitored remotely, with key assessments at 6 weeks and 4 months after delivery. Researchers will track blood pressure readings, medication side effects, and treatment adherence using therapeutic intensity scores. The study will also measure the feasibility of enrolling and retaining participants over the 4-month period, helping to inform future larger trials aimed at preventing cardiovascular disease after hypertensive pregnancy disorders.
