What this Trial Is About

Researchers are studying cytomegalovirus CMV reactivation, a common and serious issue after allogeneic hematopoietic stem cell transplantation allo-HSCT. This study focuses on adult patients who are CMV-seropositive and experience their first CMV reactivation after transplantation. The aim is to understand how CMV-specific T cell levels change during treatment and to explore their relationship with refractory or recurrent CMV infection and CMV disease. Participants will receive anti-CMV therapy chosen by their doctor, which may involve one or more approved drugs such as ganciclovir, valganciclovir, foscarnet, or maribavir. Treatment begins upon diagnosis of CMV viremia and continues until two consecutive negative CMV DNA tests spaced at least five days apart. CMV-specific T cell levels will be measured at diagnosis, three weeks after starting treatment, when treatment ends, and four weeks after stopping treatment. Patients will be monitored for 12 weeks after treatment ends. During the study, participants will have blood tests to measure their CMV-specific T cells at specified times. Researchers will track outcomes including the incidence of refractory CMV infection, recurrent infection, CMV disease, acute graft-versus-host disease, and overall survival up to 12 weeks after treatment ends. The total observation period after stopping treatment is 12 weeks, allowing detailed monitoring of immune response and disease progression.
