Researchers are conducting the CAPTURE AMI trial to compare two treatment methods for adults aged 18 and older who have experienced an ST-elevation myocardial infarction STEMI and show a high blood clot burden in their coronary arteries. This study aims to evaluate if sustained mechanical thrombectomy before stent placement reduces the amount of thrombus better than standard percutaneous coronary intervention PCI alone. The trial focuses on patients with a large thrombus burden and arteries of 3.0 mm or more in diameter. Participants will be randomly assigned to one of two groups. One group will receive standalone optical coherence tomography OCT guided PCI, while the other group will receive OCT guided PCI combined with sustained mechanical thrombectomy using the Indigo System CAT RX aspiration catheter. The study is designed as a single-center, randomized, controlled trial with two treatment arms. During the trial, participants will undergo angiographic assessment to confirm eligibility and monitor thrombus burden before stent placement. Researchers will measure the primary outcome of thrombus burden reduction at the time of stent implantation. The study will monitor participants through the procedure and assess imaging endpoints to evaluate the effectiveness of the thrombectomy device compared to standard PCI. Participation will last from the initial treatment through follow-up assessments as per trial protocols.
