Data on preventive care receipt, healthcare use, and parent experiences will be collected every 9 months for up to 5 years. The study will also explore additional preventive services, developmental delays, and economic impacts from parent, provider, and clinic perspectives. The trial is sponsored by Seattle Children's Hospital and conducted in a large primary care network serving mostly publicly insured, Black, and Latino families.

- At least one visit at the practice in the previous 9 months
- Insured by Partners for Kids, the Accountable Care Organization (ACO) for the participating network
