What this Trial Is About

Researchers are evaluating a team-based approach called the adapted PARENT model to improve well-child care (WCC) for infants and toddlers in low-income, largely Black communities. This study aims to adapt and test the PARENT intervention, which uses a community health worker as a "Parent's Coach," to provide family-centered preventive services and reduce reliance on clinicians alone. The trial will assess the impact of this approach on receiving recommended care, emergency visits, parent experiences, and economic outcomes for families and clinics. The adapted PARENT model adds a community health worker to the WCC team who independently meets families during well-child visits from newborn to 15 months of age. This coach provides anticipatory guidance, screens for social and developmental needs, and connects families to community resources. The study uses a stepped wedge randomized design where all practices start with traditional well-child care and then switch sequentially to the adapted PARENT model over time. The comparator is standard care delivered solely by primary care clinicians following national guidelines. Participants are children aged 9 to 15 months with at least one visit in the previous 9 months and insured by the associated care network. Data on preventive care receipt, healthcare use, and parent experiences will be collected every 9 months for up to 5 years. The study will also explore additional preventive services, developmental delays, and economic impacts from parent, provider, and clinic perspectives. The trial is sponsored by Seattle Children's Hospital and conducted in a large primary care network serving mostly publicly insured, Black, and Latino families.
