You may qualify if you...

  • Parents of children younger than 3 years attending a regularly scheduled primary care visit
  • Parents who answer yes to having difficulty making ends meet at the end of the month
  • Informed parental consent obtained
  • Only one child per family enrolled, prioritizing the youngest eligible child
  • For multiple births, one child will be randomly selected for inclusion
Not Eligible

You will not qualify if you...

  • Parents without legal status in Canada
  • Families currently receiving or who received system navigation support within the past year
  • Child with a diagnosed developmental disorder, genetic, chromosomal, or syndromic condition
  • Child born prematurely before 32 weeks gestation

Intervention
