Eligibility Criteria

Eligible

You may qualify if you...

  • Patients 18 years of age or older
  • Patients who underwent an amputation to any limb, regardless of location or diagnosis
Not Eligible

You will not qualify if you...

  • Patients with severe cognitive impairment or diagnosed with dementia
  • Patients deceased or in critical condition within 48 hours of the intervention
  • Patients with previous amputation to any limb
  • Patients admitted to the hospital because of a stump revision
