What this Trial Is About

Researchers are studying the relationship between the Endothelial Activation and Stress Index EASIX score and post-induction hypotension PIH in frail adults undergoing elective non-cardiac surgery with general anesthesia. Frail patients, who have impaired vascular function, may be at higher risk for PIH, a significant drop in blood pressure after anesthesia induction. This observational study aims to evaluate whether preoperative EASIX, calculated from routine lab tests, can predict PIH in this group. Participants will receive a standardized anesthesia induction including midazolam, fentanyl, propofol, and rocuronium, followed by intubation and maintenance with sevoflurane and remifentanil. The EASIX score is calculated from routine lab values of LDH, creatinine, and platelet count obtained within 24 hours before surgery, with no extra blood draws. Blood pressure will be closely monitored every 2 minutes for the first 15 minutes after induction to detect PIH. During the study, researchers will collect preoperative lab data and record mean arterial pressure for 15 minutes after anesthesia induction. They will analyze the incidence of PIH, the accuracy of EASIX in predicting it, and how frailty severity affects this risk. The study includes 160 adults aged 18 or older with a frailty score of 4 or higher. Participants usual care continues as normal, and the study completes by March 2027.
