What this Trial Is About
Researchers are evaluating the effects of a coordinated multidisciplinary team MDT approach involving hospital and primary care wound specialists compared to the usual standard care for adults with hard-to-heal ulcers. This randomized study aims to compare the number of ulcers healed between patients receiving the MDT treatment and those receiving standard care. The study follows participants for one year after enrollment to assess various clinical outcomes related to ulcer healing. Participants assigned to the MDT group receive care from an orthopedic surgeon and wound care nurses from both hospital and primary care settings. They have planned clinic visits at week 1, week 4, 3 months, 6 months, and 12 months, with additional visits as needed. Weekly wound care and bandage changes are provided by primary care nurses based on individual needs. The control group receives standard care from primary care wound nurses, with planned visits at 3, 6, and 12 months and additional visits as required. Throughout the year-long study, participants attend six fixed clinic visits with extra visits possible if necessary. Researchers monitor ulcer healing, infections, recurrence, amputations, hospital admissions, patient-related outcomes, and cost-effectiveness. Data on visits and clinical events outside planned visits are also recorded. This comprehensive follow-up includes assessments of the ulcer status and treatment effects, ensuring detailed evaluation of both care models.
