The street medicine team will attempt to contact patients within two weeks using electronic health record tools. Both groups will receive the hospital appointment line number. The study uses a non-randomized design with single masking. During the study, researchers will review health records at one and three months after the initial ED visit to measure follow-up rates, emergency department visits, hospitalizations, and mortality. Data will be collected from Denver Health, the street medicine clinic, and a health information exchange. Participants' health outcomes and care engagement will be monitored through chart reviews over the three-month period following their ED discharge.

Who Can Participate
Eligibility Criteria
You may qualify if you...
- Adult patient (18 years or older) visiting Denver Health Medical Center emergency department
- Currently unsheltered, living on streets, in a vehicle, or other non-habitable places
- Anticipating discharge from the emergency department
- Diagnosed with a condition requiring short-term follow-up, including COPD or asthma exacerbation, pneumonia, congestive heart failure with volume overload, skin and soft tissue infections, dehydration needing IV fluids, hyperglycemia from diabetes, frostbite, first- or second-degree burns, bacterial ENT infections, diabetic foot infection, traumatic head injury, pregnancy, or opioid overdose
- Has a stable location within Denver County where the street medicine team can find them for at least 2 weeks

- Altered mentation preventing communication of location, contact information, or agreement to follow-up
- Currently a prisoner
- Previously enrolled in this study

Participants receive a referral to either a rapid follow-up clinic or a street medicine clinic at time of discharge from the emergency department. The street medicine team attempts to contact participants within 2 weeks and provides primary care at the participant's location.
