Participants will be followed over a 30-month period to measure how many meet European targets for lifestyle, risk factors, and therapeutic goals for cardiovascular disease prevention. The study involves collecting data on clinical management and patient health status without administering treatments. Researchers will assess adherence to guidelines and risk management practices, helping to identify areas for improvement in preventing cardiovascular disease. The total participation time per individual varies based on the timing of their index event and follow-up assessments.

Eligible

You may qualify if you...

  • Patients aged 18 years or older at the time of identification
  • Patients who have signed an informed consent
  • At least six and at most 24 months have passed since the index event
  • Coronary patients with acute coronary events (STEMI, NSTEMI, unstable angina) or coronary revascularisation (CABG, PCI)
  • High cardiovascular disease risk patients prescribed blood pressure, lipid-lowering, or glucose-lowering treatments without existing cardiovascular disease

  • Patients living outside the defined geographical areas
  • Patients admitted from outside the geographical area or treated in non-participating hospitals
