What this Trial Is About

Severe asthma is a persistent condition marked by frequent flare-ups despite optimized treatments. Biological therapies have improved asthma control for many, yet patients often remain inactive due to symptoms and lowered exercise tolerance. This study evaluates whether a personalized exercise program can enhance exercise capacity and asthma control in adults with severe asthma who are starting biological therapy. It also looks at effects on asthma symptoms, flare-up frequency, lung function, quality of life, and body composition. Participants will be randomly assigned to two groups. The intervention group receives a tailored 6-month exercise plan created by a sports physician and physiotherapist based on initial fitness tests. This plan includes support such as phone check-ins and physiotherapy visits, plus tools like exercise diaries and step counters. The control group receives standard advice to increase physical activity without a structured program. Both groups continue their usual asthma medication, including biological therapy, managed per standard care. Participants undergo baseline fitness assessments including cardiopulmonary exercise testing, muscle strength, walking tests, and body composition measurements, along with questionnaires on asthma control and quality of life. These assessments are repeated at 6 months, with asthma control also evaluated at 12 months. The primary measure is change in peak oxygen uptake during exercise. Secondary measures include asthma symptom control, flare-up frequency, lung function, quality of life, and body composition. Data are securely managed, and participants may withdraw anytime without affecting their usual care.
