Data collection includes surveys, interviews, and quality improvement metrics to evaluate the impact of the collaborative care model in the Harlem community.

Eligible

You may qualify if you...

  • Black and Latino adults between 18 and 65 years
  • Harlem residents from low-income housing developments or receiving primary care services in Harlem
  • PHQ-4 Total Score 653, moderate risk for depression
Not Eligible

You will not qualify if you...

  • Positive screen for severe mental illness such as psychosis, mania, substance abuse, or high suicide risk
  • Those needing higher levels of mental health care will be referred elsewhere

Outpatient Treatment

Duration - Up to 24 months

Participants receive mental health task-sharing training including online education, group psychoeducation sessions, and referral support, along with supervision and participation in a learning collaborative to improve skills and coordination of care.
