Black and Latino adults between 18 and 65 years
Harlem residents from low-income housing developments or receiving primary care services in Harlem
PHQ-4 Total Score 653, moderate risk for depression
You will not qualify if you...
Positive screen for severe mental illness such as psychosis, mania, substance abuse, or high suicide risk
Those needing higher levels of mental health care will be referred elsewhere
