Researchers are evaluating a tailored cardiac telerehabilitation program designed for patients and their families to improve health literacy, health-related quality of life, family support, and patient communication with outpatient clinic nurses. The study applies a complex intervention framework combined with a patient and family participatory design, including three sub-studies: development, quantitative comparison, and qualitative investigation of patient experiences with the telerehabilitation program. Participants are divided into two groups: a control group receiving standard center-based cardiac rehabilitation care, and an intervention group receiving standard care plus an individually tailored cardiac telerehabilitation program featuring video consultations and home monitoring. The telerehabilitation program is co-created with patients, family members, and nurses through workshops to ensure it meets their preferences and needs. Family members in both groups will also receive questionnaires about support from outpatient clinic nurses. Participants will complete self-reported questionnaires at baseline and three months after the intervention, covering demographics, diagnoses, education, civil status, and patient-reported outcomes. Researchers will assess health literacy, quality of life, family support, and eHealth literacy at three months. The study monitors patient participation in cardiac rehabilitation and gathers qualitative data through observations and interviews to understand patient experiences. The study is conducted over several months, starting in May 2024 and ending in August 2027.
