Sudden Unexplained Infant Death SUID is the leading cause of death in infants between 28 days and 1 year old, with certain protective behaviors shown to reduce risk. This trial evaluates the addition of safe sleep education into the Safety in Seconds SIS mobile app, a tool previously used to teach car seat and fire safety, aiming to improve safe sleep knowledge and practices through a mobile health intervention in pediatric emergency departments PED. The study focuses on populations at higher risk for SUID, including racial and ethnic minorities and young mothers, particularly in urban areas with high infant mortality rates. Participants will be randomized to receive either safe sleep education through the updated SIS app or car seat safety education as an attention-matched control. The SIS app delivers tailored, theory-based educational messages after parents complete an assessment within the app. This study includes two groups one receiving the new safe sleep content and the other receiving existing car seat safety information. The trial will be conducted in the PED setting where families often have time and access to smartphones to engage with the app. During the study, caregivers will use the SIS app and complete surveys to measure changes in safe sleep behaviors at baseline and again one month later. Feedback will be gathered through follow-up surveys and interviews to assess the effectiveness and feasibility of using the app to deliver safe sleep education. The total duration for participant involvement includes initial app use during the PED visit and a follow-up period of one month for outcome assessment.

This table compares participation in this trial with other options available for this condition. The information is drawn from published research and the trial’s study details and is provided for educational purposes only.
| Factors | Safe Sleep Education module | Car Seat Safety Education module |
|---|---|---|
| What it is | A digital education module within the Safety in Seconds mobile app, newly developed to provide caregivers with information and strategies about safe infant sleep practices. | A digital education module within the Safety in Seconds mobile app, designed to provide caregivers with guidance on car seat safety and injury prevention. |
| Role in this trial | Tested for its effectiveness in improving caregiver-reported safe sleep behaviors for infants. | Serves as the active comparator, focusing on a separate injury prevention topic (car seat safety) for caregivers during the same encounter. |
| Primary use | Intended to educate caregivers about recommended safe sleep practices to lower the risk of infant sleep-related death, such as Sudden Infant Death Syndrome (SIDS). | Designed to teach caregivers how to correctly use car seats and understand child passenger safety to help prevent motor vehicle injuries. |
| How it is given or performed | Delivered electronically to caregivers through the Safety in Seconds mobile app on a smartphone. Users access and complete the safe sleep educational content during or shortly after a pediatric emergency department visit. | Delivered via the same Safety in Seconds mobile app platform, offering car seat safety content to be completed by caregivers during or after the emergency department visit. |
| Evidence status or supporting studies | The effectiveness of delivering safe sleep education through digital or mobile platforms is still under study, but prior educational interventions in clinical settings have shown measurable improvements in caregiver reported safe sleep practices. | Educational interventions delivered through technology have demonstrated positive effects on knowledge and self-reported behaviors around car seat safety, though mobile app-specific effects are still being investigated. |
| Important limitations | The main limitation is reliance on caregiver self-report to measure behavior change, which may be affected by recall or social desirability bias. Effects may differ outside the emergency department setting or among those with limited smartphone access. | The effectiveness relies on self-reported understanding and implementation, which may not reflect real-world behavior. Participants may be more motivated or technologically engaged than the general caregiver population. |
