During the study, participants will complete questionnaires about their ACP experiences and preferences, and researchers will track the number of participants with advance directives documented in their electronic health records at baseline and at 3, 6, and 12 months. The study involves monitoring patient-provider communication about ACP and gathering information to help implement the intervention in primary care. The total participation duration for each individual includes initial assessment and follow-up visits over one year.

Who Can Participate
Eligibility Criteria
You may qualify if you...
- Alaska Native and/or American Indian
- Age 40 or older
- Empaneled to a Southcentral Foundation primary care provider
- Diagnosed with at least 1 serious illness (e.g., cancer, COPD, kidney disease)
- Has made 2 or more visits to primary care in last 12 months
- No Advance Directive documented in electronic health record
You will not qualify if you...
- Cognitive impairment (2 or more errors) as assessed by the Short Portable Mental Status Questionnaire
- Not able to complete 30-minute study visit by phone or in person
