The trial focuses on older Alaska Native and American Indian ANAI adults, a population growing faster than the general public and facing a higher burden of serious illnesses. It evaluates a culturally tailored communication intervention called JUMPSTART-ANAI to improve advance care planning ACP discussions between ANAI patients and their primary care providers. The study also explores how to best implement this intervention in health systems serving ANAI adults with serious illness to better align medical care with patient values and preferences. Participants are assigned to one of several groups, including usual care or the JUMPSTART-ANAI intervention, which involves completing a questionnaire about ACP experiences and preferences, followed by receiving feedback with tips to help initiate ACP conversations with healthcare providers. The study compares these approaches to see which better encourages patient-driven ACP communication and documentation of advance directives in medical records. During the study, participants complete questionnaires on advance care planning experiences, communication quality, and psychological distress at various time points, including baseline, 3 months, 6 months, and 12 months. Researchers monitor the number of participants with advance directives recorded in their electronic health records. The total study period spans over two years, with assessments to evaluate both the effectiveness of the intervention and factors important for its successful implementation in primary care settings serving the ANAI community.
