What this Trial Is About

Researchers are studying advance care planning (ACP) among Alaska Native and American Indian (ANAI) adults aged 40 and older who have serious illnesses. This group faces a higher burden of illness but is less likely to use ACP, which helps ensure medical care matches patients' values and goals. The trial aims to compare usual care with a culturally tailored ACP intervention called JUMPSTART-ANAI to see which better encourages patient-driven ACP conversations and to identify factors that help implement this intervention in health systems serving ANAI adults. Participants will be assigned to one of three groups: usual care, a questionnaire about ACP experiences and preferences, or the questionnaire plus a feedback form with tips for starting ACP conversations with their primary care provider. JUMPSTART-ANAI is a behavioral intervention delivered through patient questionnaires and provider feedback during clinical visits. The study will run from April 2025 to May 2027 and includes follow-up to assess outcomes. During the study, participants will complete questionnaires about their ACP experiences and preferences, and researchers will track the number of participants with advance directives documented in their electronic health records at baseline and at 3, 6, and 12 months. The study involves monitoring patient-provider communication about ACP and gathering information to help implement the intervention in primary care. The total participation duration for each individual includes initial assessment and follow-up visits over one year.
