Who Can Participate
Eligibility Criteria
You may qualify if you...
- 12 years or older
- Diagnosis of a chronic and complex condition such as Spina Bifida, Spinal Cord Injury, Cerebral Palsy, or Traumatic Brain Injury
- Live in a community setting (not in a residential care facility)
- Pass all functional screening tests, including basic smartphone use and impairment severity assessments
You will not qualify if you...
- Diagnosis of severe intellectual disability or severe and persistent psychiatric illness
- Failing all screening tests due to severe impairments that cannot be accommodated by the mobile health system
