Completing the surveys takes about one to one-and-a-half hours. Researchers will measure patient-reported outcomes related to global health and Parkinson's symptoms over five years. Participants can skip questions they do not wish to answer and are encouraged to continue their usual care with their own providers throughout the study.

You may qualify if you...
- Diagnosis of Parkinson's disease (PD)
- Diagnosis of parkinsonism
- Diagnosis of parkinson-plus syndromes such as Multiple System Atrophy (MSA), Progressive Supranuclear Palsy (PSP), Corticobasal Degeneration (CBGD), or Dementia with Lewy bodies
- Must have access to the internet and an email address
- Must have basic computer skills
- Must be willing to complete online surveys every 6 months for 5 years
You will not qualify if you...
- Unable to read or write in English
- Unwilling or unable to complete surveys approximately every 6 months (about 90 minutes each)
