What this Trial Is About
This research evaluates ventilation methods during retrograde intrarenal surgery RIRS for kidney stones. The study compares normal tidal volume and low tidal volume ventilation under general anesthesia to see which better stabilizes the endoscopic image during the procedure. It aims to reduce movement caused by breathing that can affect laser treatment accuracy and efficiency. Participants will be randomly assigned to receive either normal tidal volume ventilation set at 6-8 mLkg predicted body weight or low tidal volume ventilation set at 4-6 mLkg predicted body weight, both with 5 cmH2O PEEP. Respiratory rate and oxygen levels will be adjusted to maintain safe breathing conditions. The study uses video analysis to measure image stability during a 60-second segment of the surgery. During the surgery, videos will be recorded and analyzed by blinded assessors focusing on the motion of the endoscopic image. Researchers will also track laser use time, surgery and anesthesia duration, breathing and heart function, medication needs, complications, and the stone-free status about four weeks after surgery. Participant safety and treatment effectiveness will be closely monitored throughout the trial.
