Eligibility Criteria

Eligible

You may qualify if you...

  • Assisted living communities with at least 40 beds
  • Communities that provide memory care services to at least 35% of residents
  • Access to hospice services from Lifepath or Suncoast Hospice
  • Residents diagnosed with dementia confirmed by a physician
  • Residents admitted to the assisted living within the last month
  • Residents have a legally responsible representative who understands English
  • Staff who are licensed nurses, administrators, or dementia care coordinators, aged 21 or older, and English speaking
  • Family members listed as legally responsible for a resident with dementia, aged 21 or older, and understand English
Not Eligible

You will not qualify if you...

  • Assisted living sites not licensed in Florida
  • Sites that previously participated in the PCEAL-DCP trial from 2018 to 2021
  • Sites with no staff willing to participate
  • Residents currently receiving hospice services
  • Residents discharged to another setting or admitted to hospice or deceased before baseline data collection
  • Staff who are not licensed nurses, administrators, or dementia care coordinators or who leave before baseline data collection
  • Family members under 21 years old or who do not understand English
  • Family members if there is already a legal authorized representative for a resident participating in the study

Outpatient Treatment
