Who Can Participate
Eligibility Criteria
You may qualify if you...
- Adolescent aged 12 to 16 years
- Attending an upcoming well-child visit with a participating provider
- Caregiver accompanying the adolescent to the well-child visit is also participating
- Both adolescent and caregiver speak and understand English
- Provider sees patients at an eligible primary care clinic
- Provider is 18 years or older
- Provider identifies as a pediatrician, nurse practitioner, or physician assistant in pediatric primary care

- Does not meet the inclusion criteria
