The Problem: The proposed trial will address the problem of how to effectively prevent subsequent falls in community-dwelling cognitively frail older adults with a history of falls. Primary Question: In community-dwelling older adults with cognitive frailty and a history of falls, can a home-based exercise program with behavioural change techniques significantly reduce falls vs. health education (i.e., control; CON)?

Who Can Participate
Eligibility Criteria
You may qualify if you...
- Aged 70 to 89 years
- Living in Greater Vancouver
- Community-dwelling (not in nursing home or extended care)
- Experienced a non-syncopal fall in the past 12 months
- Scored less than 9 out of 12 on the Short Physical Performance Battery
- Have subjective cognitive complaints
- Scored between 18 and 25 out of 30 on the Montreal Cognitive Assessment
- Able to walk independently; use of a cane is allowed
- Cleared for safe exercise by Physical Activity Readiness Questionnaire Plus and physician
- Have internet access at home
- Able to provide written informed consent
You will not qualify if you...
- Diagnosed with dementia (Clinical Dementia Rating above 0.5), neurodegenerative disease, or stroke
- Currently doing strength or balance training exercises more than twice per week in the past 3 months
- Unable to understand, speak, and read English proficiently
