What this Trial Is About
Researchers are evaluating a new approach for managing medically refractory cancer pain in patients who have not found relief through opioid medications or interventional methods. This study focuses on using radiosurgery to target specific brain areas involved in pain processing, including the pituitary hypophysis and parts of the thalamus. The goal is to reduce severe cancer pain, especially in patients with advanced disease and limited survival expectancy. Participants will receive intracranial functional stereotactic radiosurgery FnSRS aimed at ablating the pituitary hypophysis and neuromodulating the centromedian and parafascicular complexes within the thalami. This treatment delivers a high radiation dose of 90 Gy to these triple target brain regions. The study involves a specialized team including radiation oncologists, neurosurgeons, palliative care specialists, and medical oncologists. Treatment is provided in a single group without a comparison arm. During the study, participants will have baseline pain assessments over several days before treatment and daily pain measurements for 28 days after radiosurgery. Researchers will also track opioid use, side effects, and various quality-of-life measures including anxiety, depression, sleep quality, social disability, and daily functioning. The total study participation lasts about one month post-treatment, allowing detailed monitoring of pain relief and treatment safety.
