  • Adults aged 60 16 years
  • Diagnosed with chronic kidney disease stages 1 to 5
  • Owns a smartphone and is capable of using mobile applications
  • Provides informed consent
Not Eligible

You will not qualify if you...

  • Unable to provide informed consent due to neurocognitive impairment
  • Age 30 years or older
