What this Trial Is About

Researchers are evaluating the Sequential Organ Failure Assessment-2 (SOFA-2) score, a new system designed to measure the severity of organ dysfunction in critically ill adult patients in intensive care units (ICUs) across Türkiye. This updated scoring system uses routine clinical and laboratory data to reflect modern intensive care practices and aims to improve assessment of disease severity and outcomes. The study is multicenter, prospective, and observational, focusing on adult ICU patients. Adult patients aged 18 years or older admitted to participating ICUs and staying longer than 24 hours are included. SOFA and SOFA-2 scores will be calculated during the first 24 hours of ICU admission, with additional scores recorded at 72 hours for those with longer stays. There are no additional tests, treatments, or changes to standard care as part of the study. Participants will be followed from ICU admission until discharge or death. Data including demographics, admission reasons, comorbidities, and routine clinical and laboratory results will be collected. The study will assess how feasible it is to calculate SOFA-2 scores in real time and will examine the association between SOFA-2 scores and ICU mortality, comparing its performance with the original SOFA score. The overall participation duration varies depending on each patient's ICU stay.

Screening

Duration - Up to 1 day

Participants are screened for eligibility to participate in the trial.

1 visit (in-person)

Monitoring

Duration - Until ICU discharge or death, up to 60 days after ICU admission

Participants who undergo routine intensive care are observed. Clinical and laboratory data routinely collected during standard care are used to calculate SOFA and SOFA-2 scores at 24 hours and at 72 hours if ICU stay is longer than 72 hours.
