Deep infiltrating endometriosis is a severe form of endometriosis that often affects pelvic organs and is linked to infertility, chronic pelvic inflammation, and reduced success with assisted reproductive treatments. This study evaluates whether ultra-long gonadotropin-releasing hormone agonist GnRH-a pretreatment before hormone replacement therapy improves clinical pregnancy outcomes in women with deep infiltrating endometriosis undergoing their first frozen-thawed embryo transfer cycle. Participants will be randomly assigned to two groups. The experimental group will receive two injections of long-acting GnRH-a triptorelin 3.75 mg on menstrual cycle days 2 to 4 and again 28 to 30 days later, followed by hormone replacement therapy for endometrial preparation and then frozen-thawed embryo transfer. The control group will receive routine hormone replacement therapy alone before embryo transfer. The study will monitor clinical pregnancy rate and other pregnancy outcomes. During the study, participants will undergo ultrasound to detect pregnancy 30 b1 3 days after embryo transfer. Secondary measures include implantation rate, ectopic pregnancy rate, early spontaneous abortion rate, ongoing pregnancy at 12 weeks, and delivery rate. Safety will be assessed by recording adverse events after GnRH-a injections such as hot flashes and mood changes. Participants will be followed through pregnancy and delivery according to the study protocol, with total enrollment of 120 women aged 18 to 39 years.

Duration - 2 to 4 weeks
Participants are screened for eligibility to participate in the trial.
Duration - Approximately 2 to 3 months
Participants receive either ultra-long GnRH-a pretreatment followed by hormone replacement therapy or routine hormone replacement therapy alone before frozen-thawed embryo transfer.
Multiple visits for injections, hormone therapy, and embryo transfer
Duration - Up to delivery, several months depending on pregnancy progression
