  • Genetically confirmed diagnosis of 5q SMA
  • Signed informed consent to participate in the study

You will not qualify if you...

  • Diagnosis of SMA that is not 5q SMA
  • No genetic confirmation of SMA
  • Younger than 16 years old
  • No signed informed consent or withdrawal of consent
