Study on the Effect of Early Primary Care Doctor Visits After Hospital Stay in Elderly Patients and Their Risk of Hospital Readmission Within 3 Months

This research aims to understand whether an early follow-up visit with a primary care doctor within one month after hospitalization in a geriatric medicine department can reduce the risk of being readmitted to the hospital within three months. Hospital readmissions are a significant public health concern, especially in elderly patients who often have multiple health issues and are more vulnerable to complications during and after hospital stays. The study focuses on elderly patients aged 75 and older, recognizing the high medical, social, and financial impacts of frequent hospitalizations in this population. The study is observational, meaning no treatments or interventions are assigned. Participants will be grouped based on whether they have an early consultation with their primary care physician within one month after leaving the hospital or not. Researchers will collect information through short telephone calls at 1 and 3 months after discharge to ask about the patients return home and hospital readmission status. This approach helps observe the natural effects of early follow-up care without altering usual medical practices. Participants will be monitored through brief phone interviews lasting 5 to 10 minutes at one and three months after hospital discharge. Researchers will assess the impact of early primary care follow-up on rehospitalization rates, length of hospital stays, mortality at 1 and 3 months, and healthcare costs. The study does not involve direct risks or treatments but aims to provide evidence that could support better post-hospital care strategies for elderly patients. Total participation lasts about three months after hospital discharge.
