Eligible

You may qualify if you...

  • Willing to join this study for three months
  • Have limb injuries
  • Live alone in a household
  • Aged 65 years old or older
  • Independent before a traumatic injury
  • Have a smartphone
Not Eligible

You will not qualify if you...

  • Have an Injury Severity Score greater than 16 (severe injuries)
  • Have cognitive impairments
  • Diagnosed with psychiatric illness

Screening

Participants are screened for eligibility to participate in the trial.

Outpatient Treatment

Duration - 3 months

Participants receive the Geriatric Trauma Care Program (GTCP) through a nurse practitioner-led, digitalized intervention alongside routine care to support recovery from traumatic injuries.

Multiple visits during the 3-month treatment period

Follow-up

Duration - Up to 3 months after hospital discharge

Participants are monitored for pain intensity, functional disability, depression, loneliness, and health-related quality of life after completing the treatment period.
