Researchers are conducting an observational study to assess patients who have experienced athero-thrombotic events such as coronary artery disease (CAD), cerebrovascular disease (CVD), or peripheral artery disease (PAD). This study aims to evaluate how well patients follow guidelines for managing cholesterol levels and other important modifiable risk factors to help reduce the chances of future cardiovascular events. The study is carried out across multiple cardiology centers in Italy and includes several phases. It begins with an educational intervention discussing secondary prevention guidelines, followed by data collection over 3 months using a web-based form to track adherence to these guidelines and reasons for non-adherence. After a 6-month evaluation, a second educational intervention is conducted to review results and address gaps. Another 3-month data collection and 6-month evaluation follow, with a final 12-month follow-up of all participants from both enrollment phases. Participants will provide data through the web Case Record Form and will be monitored for adherence to cholesterol and other risk factor guidelines. The research team will evaluate these adherence rates at multiple points, focusing on improving patient care. The total participation time includes initial phases and a 12-month follow-up, with ongoing data collection and educational support throughout the study.

Eligibility Criteria
You may qualify if you...
- Both sexes at birth, age 18 years or older
- Documented coronary artery disease, including prior acute coronary syndrome, coronary artery bypass graft, or percutaneous coronary intervention
- Documented peripheral artery disease with evidence such as prior peripheral bypass surgery, angioplasty, amputation, intermittent claudication, or Ankle Brachial Index less than 90
- Documented cerebrovascular disease including ischemic stroke or previous carotid vascular interventions
- Signed informed consent

- Active cancer or very severe disease limiting short- to medium-term life expectancy
- Participation in other interventional clinical studies
- Prior enrollment in this study at another center or in a previous phase
