Eligible

You may qualify if you...

  • Identify as African American or Black
  • Have two eGFR tests below 29 separated by at least 90 days within the past 2 years, or a Kidney Failure Risk Equation score of 10% or greater for kidney failure within 2 years
  • Had an emergency room or hospital visit at Emory Healthcare within the previous 2 months
  • Willing to comply with all study procedures and available for the study duration
  • Age 18 years or older
Not Eligible

You will not qualify if you...

  • Currently on dialysis
  • Receiving hospice care or conservative management for terminal illness
  • On waitlist, referred for, or completed a transplant evaluation within the past 2 years
  • Have had a kidney or other solid organ transplant
  • Undergoing active cancer treatment
  • Non-English speaking
  • Participating in another treatment or intervention study at enrollment
  • Currently pregnant or planning pregnancy at recruitment

Treatment

Duration - 6 months

Participants in the intervention group receive patient-centered kidney health coaching with support from trained kidney health coaches over six months. This includes an initial rapport-building call, ongoing telephone support at least twice a month, attending in-person clinic appointments, and assistance with self-management and treatment decisions. Participants in the control group receive usual care based on their healthcare setting without coordinated kidney health coaching.

Follow-up

Duration - Up to 12 months after treatment start

Participants are monitored for up to 12 months to assess kidney replacement therapy initiation, survival, symptom burden, quality of life, and other health outcomes.
