What this Trial Is About

Researchers are conducting a prospective, randomized, single-blinded controlled trial to compare computed tomography CT-guided and conventional methods for selecting the size of double-lumen tubes DLT in patients undergoing thoracic surgery that requires one-lung ventilation. The study aims to determine if using preoperative CT-based bronchial diameter measurements improves the accuracy of the initial DLT size selection compared to traditional methods based on demographic information. Participants will be randomly assigned to one of two groups. In the CT-guided group, the bronchial diameter is measured 10 mm distal to the carina using preoperative CT scans, and the DLT size is chosen based on manufacturer-recommended charts derived from these measurements. The conventional group selects DLT size using standard clinical criteria such as sex, height, and weight, following institutional tables. During surgery, researchers will assess whether the correct DLT size was chosen on the first attempt using a fiberoptic scope to confirm optimal position and lung isolation. They will also monitor the need for tube replacement, time to successful intubation, occurrence of hypoxia, airway pressure changes, surgeon satisfaction with lung isolation, and post-operative throat discomfort or airway injury within 24 hours after extubation. The total participation covers the intraoperative period and immediate post-operative assessments.
