Researchers are evaluating whether home-based cardiac rehabilitation combined with a family empowerment approach can help patients recovering from acute coronary syndrome (ACS). The study aims to find out if this method improves health-related quality of life and peak oxygen consumption (peak Vo2) better than usual care. The trial includes adult patients aged 18 to 65 who have recently experienced ACS and have been discharged from the hospital between 3 months and 1 year prior to enrollment. Participants in the experimental group will undergo a 12-week home-based cardiac rehabilitation program that includes physical exercise, dietary management, smoking cessation, and stress management guided by educational materials and support from family and healthcare professionals. The intervention also involves regular monitoring of vital signs and exercise intensity based on a 6-minute walk test. The control group will receive usual care with education on medication adherence, exercise, smoking behavior, diet, and stress management in a single session. During the study, participants will perform prescribed walking exercises, follow diet and stress management plans, quit smoking, and comply with medication. They will visit a cardiologist at weeks 1, 6, and 12, and receive ongoing support from a nurse facilitator through video calls or home visits. Researchers will assess health-related quality of life and peak Vo2 at enrollment and after 12 weeks. Daily reports on diet, exercise, medication adherence, and heart rate during exercise will be collected and monitored to evaluate progress and safety.

Participants undergo baseline measurements including a 6-minute walk test with ECG before and after to determine exercise intensity. Patients and families receive educational materials and videos about disease, treatment, stress management, diet, and smoking cessation.

Duration - 1 to 2 weeks
Participants and their families receive education on acute coronary syndrome, cardiac rehabilitation, risk factor control, and family roles. Facilitators help improve self-efficacy, self-esteem, and family participation through group sessions with feedback and Q&A.
1 to 2 group sessions (in-person or video)
Duration - 12 weeks
Participants follow a 12-week home exercise program with physical exercise sessions 3 times a week. Families monitor heart rate and symptoms daily and report diet, exercise, and complaints through forms. Counseling on smoking, diet, and stress management is provided initially with follow-up by nurse every 2 weeks in month 1 and every 4 weeks thereafter. Cardiologist visits occur at weeks 1, 6, and 12.
