You may qualify if you...
- Patients diagnosed with type 2 diabetes referred to endocrinology due to poor metabolic control
- Patients currently receiving insulin therapy
- Adults aged 18 years or older
You will not qualify if you...
- Referral from primary care for reasons other than poor metabolic control, such as hypoglycemia
- Patients with other types of diabetes mellitus or uncertain diabetes classification
- Patients on rapid-acting insulin bolus therapy
- Patients experiencing acute decompensation of diabetes
- Patients receiving corticosteroid treatment
- Individuals dependent on others for daily activities with a Barthel index below 60 or with cognitive impairment
- Patients with a language barrier
