During the study, participants will be evaluated through various physical tests and questionnaires both during hospital admission and after discharge. Researchers will track functional abilities, balance confidence, and physical activity levels to determine if discharge recommendations were appropriate. The study involves monitoring physical and functional status to better understand recovery and risks post-discharge, with the aim to support safe transitions back home for older adults. Participation lasts from hospital admission through at least two weeks post-discharge.

Eligibility Criteria

Eligible

You may qualify if you...

  • Aged 65 years and older
  • Able to walk at least 5 metres without physical help, with or without walking aids, as before hospital admission
  • Clinical Frailty Scale score of 5 or less
  • Able to understand and follow instructions with an Abbreviated Mental Test score of 5 or higher
Not Eligible

You will not qualify if you...

  • Admitted for acute orthopedic or neurological conditions that affect mobility
  • Requires physical assistance from one or more persons to move
  • Clinical Frailty Scale score greater than 5
  • Abbreviated Mental Test score less than 5, indicating lack of capacity to consent
