This research aims to evaluate the effectiveness and spread of digital health care delivery models in improving health outcomes among the Medicaid population. The study focuses on patients with hypertension, diabetes, and maternal health conditions. It uses a mixed methods design including a controlled interrupted time series and cohort approaches, as well as interviews with patients and providers to understand their experiences. Participants are drawn from OSF HealthCare and several federally qualified health centers participating in the Medicaid Innovation Collaborative (MIC) program. The intervention studied is a digital health delivered multicomponent program that ensures minimum connectivity standards for the conditions under study. The study includes various groups from different health systems enrolled in the MIC program. During the study, participants will be monitored at multiple points to assess changes in blood pressure control, blood sugar control, and patient assessments of chronic illness care and health care management. Patient satisfaction will also be measured at program exit. Data collection will include longitudinal interviews, and assessments will occur at baseline, 3, 6, and 12 months, with follow-up averaging one year. The study period extends until 2028, allowing thorough evaluation of outcomes and experiences.

Visits at 3 months, 6 months, and 12 months to assess health outcomes
Duration - On average 1 year
Participants are followed longitudinally to study their experiences and changes in health care management and satisfaction.
