What this Trial Is About

Researchers are evaluating the use of high-density flexible microelectrocorticography ECoG to guide surgery in patients with drug-resistant focal epilepsy. This study aims to compare surgical outcomes with and without intraoperative ECoG guidance and to explore electrophysiological markers recorded during surgery that may predict surgical success. The study involves patients aged 3 to 60 years who have persistent seizures despite medication and are considered suitable for resective epilepsy surgery. During surgery, a sterile high-density flexible ECoG device is placed on the exposed brain surface over the suspected epileptogenic zone and nearby areas. This device records detailed brain activity in real time, including interictal spikes and high-frequency oscillations, to help surgeons define the epileptogenic zone and decide the extent of tissue removal. The ECoG array is used only during surgery and removed before wound closure. Participants will be followed for up to one year after surgery, with assessments including seizure frequency and severity, cognitive and developmental function, quality of life, and monitoring for complications or adverse events. Primary outcomes focus on seizure freedom six months after surgery. Evaluations occur at baseline, six months, and one year postoperatively to understand the impact of ECoG guidance on surgical prognosis and patient well-being.
