- Patients being discharged home after elective colorectal surgery
- 18 years or older
- Able to speak and understand English or French
- Provide informed consent
You will not qualify if you...
- Post-operative hospital stay less than 3 days
- Require discharge to a rehabilitation centre before going home

Your Study Journey

Participants are screened for eligibility to participate in the trial.

Participants undergo elective colorectal surgery and receive usual post-operative care in hospital.
Daily in-hospital monitoring until discharge
Duration - 30 days following hospital discharge
Participants in the intervention group use a mobile app to complete daily health checks and receive tailored recovery support after discharge. The control group receives usual follow-up care.
Daily health check emails on Days 1-14, 21, and 30 post-discharge
Duration - 4 weeks after hospital discharge
Participants complete assessments of quality of recovery and self-efficacy to manage their own health care.
