What this Trial Is About
This research focuses on homeless-experienced Veterans undergoing housing transitions and evaluates the implementation of Critical Time Intervention CTI, a structured and time-limited case management practice. The study addresses the current variation in case management approaches across VA Grant and Per Diem GPD case management aftercare sites by implementing CTI at 32 sites nationwide. The goal is to assess how well CTI can be integrated and sustained within these programs.
Participants in this study are involved in one of three groups receiving a stakeholder-informed training and technical assistance strategy called Replicating Effective Programs REP, REP combined with site-specific weekly external facilitation to support CTI implementation enhanced REP, or no CTI implementation as a control group. The study uses the REP implementation bundle to support CTI at all sites, with half receiving additional facilitation for nine months. This is a type 3 hybrid trial designed to compare REP versus enhanced REP methods.
Throughout the study, researchers will monitor how faithfully CTI is implemented and maintained over time, as well as the costs involved in delivering CTI and the implementation strategies. They will also evaluate Veteran housing stability, hospitalization rates, outpatient service use, and the financial return on investment linked to CTI. The study spans up to four years, with assessments at intervals including 6, 12, and 18 months after CTI implementation starts, aiming to develop resources to help sustain CTI in the future.

Factors
REP Implementation Strategy
Enhanced REP (REP + External Facilitation)
Control Group (Implementation as Usual)
What it is
A stakeholder-informed implementation strategy providing training and technical assistance (REP) to support site staff in delivering Critical Time Intervention (CTI) case management.
An enhanced implementation approach combining REP with additional site-specific support (External Facilitation) to help sites tailor CTI adoption and address local barriers.
Sites receive usual VA procedures for implementing CTI without the structured REP or facilitation supports.
Role in trial
Assigned to select GPD aftercare sites to compare fidelity and sustainment of CTI delivery.
Assigned to sites as a comparator to standard REP to test if added facilitation leads to improved CTI implementation and sustainment.
Represents how CTI implementation occurs at sites receiving routine support only.
Primary use
Supports site teams in consistently adopting CTI for homeless-experienced Veterans as part of VA GPD aftercare.
Aims to increase CTI implementation with greater site-tailored problem solving and change management support.
Allows comparison to structured implementation approaches by observing usual CTI adoption without added strategy.
How it is given or performed
REP provides structured CTI training, materials, and periodic technical assistance to assigned site staff.
Sites receive the full REP package plus hands-on, local, external facilitation sessions designed to actively address local implementation challenges.
Sites continue routine CTI practice with only standard information and resources, as is usual care for GPD aftercare programs.
Evidence status
The REP model is based on an established implementation framework, with this trial collecting evidence on its effectiveness for CTI delivery among VA GPD sites serving homeless-experienced Veterans.
Enhanced REP is under evaluation in this trial to determine if external facilitation increases successful CTI implementation and sustainment beyond structured technical support alone.
Serves as the baseline for comparative effectiveness, representing standard practices without additional structured implementation support.
Important limitations
Effectiveness may vary depending on organizational context and local staff engagement, and results may not be generalizable to non-VA or non-GPD aftercare settings.
The added resource of external facilitation may not be feasible or scalable in all programs. Results may depend on individual site needs and facilitator capacity.
Sites may have limited support and guidance, potentially leading to inconsistent CTI implementation that does not reflect best practices.
