Researchers are evaluating whether using the iCAGES (integrated CAncer GEnome Score) tool to guide treatment for advanced cancers is better than current standard care or immunohistochemistry (IHC)-guided therapy. The study focuses on patients with advanced lung cancer or other advanced cancers, aiming to compare progression-free survival, overall survival, and quality of life among different treatment approaches. This observational study explores personalized cancer treatment based on genetic analysis and prior research knowledge. The study has three groups: one receives treatment guided by whole exome or genome sequencing analyzed with the iCAGES system to prioritize targeted drugs, another receives treatment following standard guidelines without iCAGES analysis, and a third group receives treatment based on IHC detection of multiple protein molecules in tumor tissue. Targeted drugs such as inhibitors and antibody-drug conjugates (ADCs) like tarceva, capmatinib, and padcev may be selected for the iCAGES and IHC groups. Treatments are chosen based on genetic and protein testing results, expert review, and drug availability. Participants will be monitored for progression-free survival and overall survival over two years, along with quality of life assessments and pain evaluation up to one year. The study collects clinical and imaging data from hospital records, including PACS images. Researchers will compare outcomes between groups to assess the benefits of iCAGES-guided therapy. The total study duration and follow-up last up to two years per participant, with ongoing observation of survival and quality of life measures.
